A 35 year old male with Acute Gastroenteritis..

A 35 yr old male came with c/o loose stools since 3 days & syncopal attack 1 episode 6hours back & vomiting 1 episode
Pt was apparently normal 3 days back,then developed loose stools daily upto 3-4 episodes, which was subsided in 3 days followed which the patient was normal up to 10 hours,again started developing abdominal pain the patient went to local RMP and took some medication and followed which the patient developed itching all over the body and then had 1 episode of syncopal attack for about 3-5min,followed which pt was taken to nearest government hospital and 3 units fluids given in view of Hypotension and was referred to KIMS for further evaluation..
No H/O fever, palpitations
No H/O DM, HTN, TB, ASTHMA, EPILEPSY

O/E :
On examination patient is conscious, coherent, co-operative
Pallor present 
No icterus, cyanosis, clubbing, lymphadenopathy pedal Oedema
Vitals
Temp: Afebrile
BP:90/60mmhg
PR:100bpm
RR:20/min
SPO2-96%
GRBS-253

Systemic Examination :
Cvs - S1S2 +
RS -BAE+
P/A- soft, tenderness in the umbilical region
No organomegaly
Bowel sounds heard.
CNS-NAD

Investigations :
Provisional Diagnosis
Acute Gastroenteritis,with persistent hypotension..

Treatment :
IVF 1 unit NS & RL
Inj.Pan 40mg po/od
Inj.zofer 4mg Iv/tid
Tab.sporolac Ds po/tid
Inj.optineuron 1amp in 100ml iv/od
